Fro tourist visa if you are applying make it address to the consul and with address of the embassy you are applying, take note that your letter should be stated why you are asking for a visa

Otherwise just write:

Dear Sirs,
Please send me a Tourist Visa application form.
Yours faithfully

don't forget to put your address, sign and date it.

Note some countries you just get a visa stamped in your passport as you go through arrivals after filling out a form on the airplane.   Check with your travel agent so you do not waste needless time.
