Firstly, if you live in any foreign country (except Canada) you must renew your passport by visiting the US consulate or embassy nearest to your place of residence. For those residing in the US or Canada, keep reading:
Renewing a US passport For people residing in Canada or the United States, you can conveniently submit your passport renewal forms by mail to:
National Passport Processing,
P.O. Box 13349, Philadelphia,
PA 19101-3349.
In this case, you must use form DS-82 which you can download from the US Department of State's website.
Applying for a new American passport The site also has a lot of useful information about your renewal application that you might find useful, so I'd recommend having a quick read.
The application fee is currently $67 and this must be paid at the time of application. If you are mailing in your request, you must include a cheque for this amount payable to the Department of State.
You must also include two passport photos, as well as your expired (or soon-to-expire passport).
If you are in a rush to get your new passport, the State Department website also has an expedited service (details of which are on their site). There is also a useful breakdown of application times. Very handy if you're planning to travel in the near future!
